Insurance 

If you will be using an insurance benefit for your services, you will need to call your insurance carrier to verify benefits and obtain coverage information. Verification of benefits is not a guarantee of coverage. You as the customer are ultimately responsible for verifying your own coverage, obtaining authorizations if needed, and paying deductibles, co-insurance, or co-pays as dictated by your plan.

How do I contact my insurance company?

You should see a "Members Services" or "Customer Service" number on your insurance card. The number is typically on the back of your card. When speaking with your insurance, be prepared to share your ID number, name, date of birth, and social security number.

Cancellation Policy 

Our general policy is that we receive a notice of cancellation either through email or phone within 24 hours of you knowing that you will not be able to attend the scheduled session. In the event that we do not receive notification with a 24 hour period. A $50 late fee will be charged for the missed session. If there is an emergency such as a medical issue, natural disaster, etc... a late fee charge is not incurred.

This notice describes how your health information may be used and disclosed and how you can get access to this information. If you are actively engaged in therapy services, you also have the right to request a copy of our privacy practices at any time. 

No Surprises Act 

Under Section 2799B-6 of the Public Health Service Act, you are entitled to receive a "Good Faith Estimate" of the charges for your treatment if you are uninsured or using a self-pay option. Due to the individual circumstances of each person. The total cost of counseling will vary depending on the number of sessions. In the event you receive a bill for services in excess of $400 of the "Good Faith Estimate" you can dispute or appeal the bill.
